Forms
What to do?
1) download the appropriate form(s)
2) fill out the form on a computer or by hand (date, information about you, Director of studies*, the main body of the request, attachments**)
3) sign by hand
4) submit to our IRO office (building A, room 120, usually Monday-Friday 9:00-14:00)

**attachments: only obligatory when requesting an IOS (individual study plan outline) or a study leave (depending on your case). Submit the attachment(s) with the request form, please.
